New Patient Registration
The Practice area is in the southern part of the city, bounded by the three bridges: Folly Bridge, Donnington Bridge and Red Bridge. Existing patients who later move to the outer boundary (Argyle Street, Bedford Street, Chester Street, Donnington Bridge Road, Fairacres Road, Freelands Road, Jack Daw Lane, Meadow Lane, Parker Street, Radcliffe Road, Stratford Street, Swinburne Road and Warwick Street) may remain registered.
Since April 2004 patients are registered with the Practice rather than with an individual GP.
We ask all new patients to provide photo identification and proof of their address (recent utility bill or tenancy agreement) and to complete a medical questionnaire and registration form. Both of these forms are available to download from the above "Pre-Registering Online" page. New patients are offered a health screen that includes a review of past and present medical history, blood pressure, weight, height and family history.
Medical treatment is available from the date of registration. Please contact reception for further information.
*Please note due to increased workload, we are not able to take on out of area patients at the moment. The GP Partners hope to review this in the future. We apologise for any inconvenience this may cause.
Change of Address
Please notify us as soon as possible of any change of address or telephone number. This prevents problems when sending you letters, results, making hospital appointments and home visiting.
